Eliminating health disparities requires a commitment to collecting meaningful patient demographic data that healthcare organizations can use to better understand their patient populations and community, and assess their needs. The HEOA can help identify opportunities for improvement in an organization’s ability to collect, validate, and stratify patient demographic data, and implement and monitor interventions to address disparities.
There is still time for your facility to complete an HEOA assessment and receive a report categorizing your facility’s results. Please contact your QIN-QIO or HQIC QI Advisor for more information.
